At Horton Orthodontics, we are proud to accept new patients without a referral! That means you can just request an appointment when you or your child is in need of an orthodontic evaluation. If you are an existing patient or a patient with Medical Assistance (MA), MNSure or Medicaid, we kindly ask that you contact our office to make an appointment with one of our friendly team members as this form is intended for new patient consultations only.
